Worker's Compensation Policy
If you are being seen for a worker's compensation injury, our office must have written approval from your employer, nurse case manager, or insurance adjuster prior to your appointment date. If this information is not received, your appointment will be rescheduled.
Referrals
If your insurance requires a referral to see a specialist, we must have this by your appointment time or we will need to reschedule your visit for a later date. You can have your physician fax the referral to our office at (757) 461-8967. If you are not sure if your insurance requires a referral, please contact the Member Services phone number on your insurance card so that they will be able to determine this for you.
